When referring a patient, wherever possible please include:
If you’re ordering bloods and referring a patient to see Beck, the following are useful to include (appropriate to condition):
Please note, Beck does not offer bulk billing, so there will be out of pocket / gap expenses. All patients (except DVA or self-managed NDIS) are required to pay the full fee at the time of their appointment, and they will receive any benefits from Medicare following their appointment. - Reason for referral
- Latest blood results
- Current medications
- Relevant past medical history
- Chronic Disease Management Plan and referral form
- Eating Disorder Treatment Plan, if applicable – GP template and tools
- If your patient is on insulin, please complete and return an Insulin Titration Authority Form
If you’re ordering bloods and referring a patient to see Beck, the following are useful to include (appropriate to condition):
- Full Blood Count (FBC)
- Liver Function Test (LFT)
- HbA1c
- Blood glucose (fasting and/or OGTT)
- Insulin (fasting and/or insulin curve if doing OGTT)
- C-Peptide and GAD/IA2 antibodies
- Cholesterol and lipids
- C-Reactive Protein (CRP)
- Hormones i.e. thyroid, sex, adrenal (cortisol/DHEA-S)
- Iron studies
- Kidney function (C,U&E)
- Vitamins and minerals – Magnesium, Phosphate, Potassium, Active Vit B12, Vit D (25-OHD), Folate
