Referral information & fees
Patients do not need a referral to see Beck.
For health professionals referring a patient, wherever possible, please include:
Please note that Beck sees people aged >16 years and does not offer bulk billing, so there will be out-of-pocket / gap expenses. All patients (except DVA) are required to pay the full fee at the time of their appointment, and they will receive any eligible benefits from Medicare following their appointment. Beck can no longer accept patients who are NDIS Plan-Managed, only patients who are Self-Managed and who are able to pay at the time of their appointnet and seek re-imbursement from NDIS.

- Latest blood results
- Current medications
- Relevant past medical history
- Care Plans:
- GP Chronic Condition Management Plan (GPCCMP) - previously called Chronic Disease Management Plan (CDMP), if applicable
- Eating Disorder Treatment Plan, if applicable – GP template and tools
